Your first visit
To ensure your first appointment proceeds smoothly, plan on arriving 15 minutes early to complete needed forms.
Bring a copy of any medications, you doctor's phone number, your insurance card and any recent dental x-rays.
We are happy to request x-rays from your previous dental office. Just call that office to request the transfer.
Let us know if you need to pre-medicate for your visit, by order of your physician.
We will review your dental and medical history and current needs.
We will perform a complete exam of your mouth including a soft tissue exam, periodontal probing, dental charting and radiographs if necessary.
